If you do not agree with any part of these Terms, you must not use our website.

1. Website for Informational Purposes Only

This website provides general information about our counseling and psychotherapy services. The content, including articles, blog posts, and service descriptions, is for informational purposes only. Accessing this website or submitting information through any form on this website does not establish a therapist-client relationship. A professional relationship is only formed after you have completed our intake process, signed a formal client agreement, and have been officially accepted as a client of our practice.

2. Not for Emergency Use

This website is not a crisis or emergency service. If you are experiencing a mental health crisis, a medical emergency, or are having thoughts of harming yourself or others, please do not use this website or its contact forms.

  • Call 911 immediately.

  • Go to the nearest hospital emergency room.

  • Call or text the 988 Suicide & Crisis Lifeline at 988.

We are not equipped to handle emergencies through this website and do not monitor communications 24/7.

3. Privacy and Confidentiality

Your privacy is critically important to us. Our use of any information you provide via this website is governed by our Privacy Policy. Please review it to understand our practices.

Please be aware that communication via standard email or website contact forms is not completely secure. While we take measures to protect your information, we cannot guarantee the confidentiality of information transmitted through these means. Please avoid sending sensitive or detailed personal health information through our general contact forms.

Our practice adheres to the privacy and security rules of the Health Insurance Portability and Accountability Act (HIPAA).

4. Client Rights and Protections

A. Notice of Privacy Practices (HIPAA)

The Health Insurance Portability and Accountability Act (HIPAA) is a federal law that provides privacy standards to protect your medical records and other Protected Health Information (PHI). It specifies how we can use and disclose your PHI.

Our detailed Notice of Privacy Practices (NPP) explains your rights under HIPAA, our legal duties to protect your PHI, and how we may use and disclose your information. You have the right to review our NPP before signing any client agreement. A copy will be provided to you during the intake process.

By law, we are required to:

  • Keep your Protected Health Information private.

  • Provide you with this notice of our legal duties and privacy practices.

  • Follow the terms of the notice that is currently in effect.

B. Your Right to a Good Faith Estimate (No Surprises Act)

Under the No Surprises Act, you have the right to receive a “Good Faith Estimate” explaining how much your mental health care will cost.

This applies to you if you are uninsured or if you are not using your insurance for our services (i.e., you are a private pay client).

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency services.

  • We will provide you with a written Good Faith Estimate at least 1 business day before your scheduled service. You may also ask for an estimate at any time before scheduling.

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

  • Make sure to save a copy or picture of your Good Faith Estimate for your records.

5. Eligibility and Use of Services

Our services are intended for individuals who are 18 years of age or older. If you are a minor seeking services, you must have the consent of a parent or legal guardian. All services are provided in accordance with the laws of the State of Ohio and the ethical guidelines of our licensing boards. Our therapists are licensed to practice in Ohio, and services are generally limited to residents of Ohio.

6. Appointments and Cancellation Policy

Appointments can be requested through our secure client portal or by calling our office. Our cancellation policy, which requires at least [e.g., 24 or 48] hours' notice to avoid a cancellation fee, will be detailed in your client services agreement.

7. Intellectual Property

All content on this website, including text, graphics, logos, images, and articles, is the property of Susanne Perry Counseling, LLC or its content suppliers and is protected by United States and international copyright laws.
